Humanizing Kidney Care
Core to the Coalition's mission is the integration of palliative and kidney care to create a truly supportive, patient-centric model that improves long-term quality of life and end of life decision-making. The complexities of modern healthcare can leave patients with overwhelming treatment decisions, and supportive care models can ensure patients are informed and engaged.

Informing Clinical Practice and Health Policy
The Coalition's subject matter experts and advisors, including its Executive Committee, conduct impactful research to find new and innovative ways to improve kidney supportive care.
- The ExPAND Project
Expanding and Promoting Alternative Care and Knowledge in Decision-Making (The ExPAND Project), a 5-year study funded by the Patient-Centered Outcomes Research Institute, is focused on improving shared decision-making and access to non-dialytic treatment for people with kidney disease.
- The HIGHway Project
Honoring Individual Goals and Hopes (The HIGHway Project) empowered social workers and nurse care managers in dialysis centers to incorporate advance care planning into their workflows, addressing an unmet desire by patients with kidney failure to have conversations with their providers about their end-of-life wishes.
- The MY WAY Study
Make Your Wishes About You (MY WAY) implemented coaching in chronic kidney disease (CKD) clinics to improve patient engagement in advance care planning.
- The Pathways Project
The Pathways Project team developed a change package with best practice recommendations for instituting a kidney supportive care program, and then worked with a collaborative of approximately 15 organizations to implement the programming.
