The Coalition has released new resources for clinicians, patients and caregivers! Our team
has proudly led several major kidney supportive care projects in recent years, including:
- MY WAY
Make Your Wishes About You (MY WAY) implemented coaching in chronic kidney disease (CKD) clinics to improve patient engagement in advance care planning. - HIGHway
Honoring Individual Goals and Hopes (The HIGHway Project) empowered social workers and nurse care managers in dialysis centers to incorporate advance care planning into their workflows, addressing an unmet desire by patients with kidney failure to have conversations with their providers about their end-of-life wishes. - Pathways
The Pathways Project team developed a change package with best practice recommendations for instituting a kidney supportive care program, and then worked with a collaborative of approximately 15 organizations to implement the programming.
During each of these projects, the research team generated guides, toolkits, change packages and more aimed at helping patients, caregivers, and clinicians through the programming. We have made many of these resources available on the Coalition's website.
